Looks like either calcite or quartz that has grown in a fault gouge, crystallizing in a pocket between the handingwall and footwall, changing orientation as the fault moved.

Looks like an epimorph or perimorph of something. Find a broken crystal and see if you can work out the form of the mineral the epimorph covered

Not really.

It may help to know where it was found, but I think I can say with certainty that it isn't a sand rose of any sort, ancient or otherwis ...
